The Health Impact Project is seeking proposals from partner organizations who work nationally, regionally, or at the state level focused on implementing systems and policy change to improve social determinants of health and health equity. At least four organizations will be selected and receive up to $45,000 to carry out initiatives over a maximum of 18 months with an aim to sustainably integrate health considerations into systems and policies.
