The NACCHO Health and Disability Program is seeking success stories from local health departments (LHDs) on ways the disability community has been highlighted as a part of community health assessments and/or community health improvement plans. These stories will be used to create a “best practices” resource designed to encourage LHDs to engage individuals with disabilities within such efforts.
