The Merck Foundation recently shared the results of the HIV Care Collaborative (HCC) initiative aimed at helping communities better serve their HIV-positive (HIV+) populations. From 2012 to 2015, the Foundation committed $3.6 million to support local health departments in Atlanta, Georgia; Houston, Texas; and Philadelphia, Pennsylvania and a National Program Office at the George Washington University Milken Institute School of Public Health. Each program site helped identify and connect HIV+ individuals to appropriate care and treatment. The HCC model charted a path for effective integration of innovative, community-based approaches with public health systems to improve timely access to high-quality HIV care for underserved populations. These efforts helped promote improvements in linkage to HIV medical care and yielded insights into effective strategies to support better community care linkages nationwide.
