Participatory disease surveillance systems rely on voluntarily submitted symptom data from the general public. Please help us understand the role of participatory disease surveillance in your current work or the potential for it to supplement traditional surveillance systems by completing this brief survey by 5:00PM ET on June 8. It should take no longer than 5 minutes to complete and may help guide the development of surveillance tools to maximize their utility for local health departments.
