The HIV Care Collaborative for Underserved Populations, supported by Merck Foundation, works closely with community partners to connect newly HIV+ individuals to high quality treatment and reconnect other clients lost to care. - Fulton County, GA: The Bridging the Gap Initiative is operated by the Fulton County, GA Department of Health and Wellness. The goal is to help improve the health outcomes of the more than 33,000 Fulton County HIV+ residents.
- Philadelphia: The HCC’s partnership with the Philadelphia Department of Public Health and ActionAids makes a difference in Philadelphia. The Engaging HIV+ Patients in Care Initiative helps to improve access to HIV medical care and community-based services for underserved HIV+ individuals in Philadelphia.
